Developing infection prevention and control guidelines for healthcare professionals in the Netherlands: an evaluation of the process

背景

医療における院内感染症は、患者にとって生命を脅かし得るリスクをもたらし、医療コストの増大に拍車をかける可能性がある。この問題に取り組むため、Dutch Collaborative Partnership for Infection Prevention Guidelines（SRI）は、病院、長期ケア施設（LTCF）、および公衆衛生環境における感染症を減少させるためにエビデンスに基づくガイドラインを作成している。

目的

現行の作成手順の実施可能性に関する洞察を得る目的で、エビデンスに基づくガイドラインの作成プロセスに関する医療専門家の経験を評価すること。

方法

ガイドライン作成グループのメンバーを対象に、2021 年から 2022 年にかけて調査を行った。参加前の期待、経験した作業量、ガイドライン作成グループの構成に対する満足度、ガイドラインの作成プロセス、ならびに全般的またはドメイン別のガイドライン、ならびに実施に関する因子に関するデータを収集・分析した。

結果

ガイドライン作成グループ 17 グループのメンバー 168名中 80 名（48％）が調査に回答した。46 名（57％）は参加前の期待が明らかであったと回答した。27 名（34％）は、時間投資が予想よりも多かった、特に文献スクリーニングにおいて多かったと回答した。70 名（88％）は、自分たちの組織が十分に代表されていたと回答し、69 名（86％）は、感染予防に関する十分な知識が存在していたと回答した。しかし、25 名（31％）は、Grading of Recommendations Assessment, Development and Evaluation（GRADE）は現行のエビデンスには適していないが、代替は存在しないと回答した。32名（40％）は、全般的なガイドラインをドメイン別のガイドラインに改訂してほしいと回答した。

結論

回答者は、全般的なガイドラインのドメイン別のガイドラインへの改訂の必要性を強調しており、このことは当該分野の必要性に密接に沿ったガイドラインの作成が必要であることを示唆している。改善の必要がある領域、例えば作業量の管理、方法論に関する懸念、および実施のための戦略などの領域に対処することは、作成プロセスを最適化し、ガイドラインが感染予防に確実に影響を及ぼす上で不可欠である。

監訳者コメント：

オランダでは 2021 年に新組織 SRIが 設立され、病院・長期ケア・公衆衛生の全領域を対象とした「汎用ガイドライン」の作成を開始している。ガイドライン作成には多くの時間が必要であったが、できあがったガイドラインの質は高いと回答されている。一方で、汎用ガイドラインでだけでなく、領域別ガイドラインの必要性やガイドライン作成プロセスの評価や改善が重要との意見もあった。
