Comparison of cleaning effects of vacuum-assisted boiling washer-disinfectors on rigid endoscopic instruments: impact of cleaning cycle parameters

背景

真空沸騰洗浄消毒器は、硬性内視鏡の洗浄において、広く支持されているが、標準的な洗浄サイクルは、時間がかかるので、機器の回転率に影響を与える。

目的

パラメーターの異なる洗浄サイクルの洗浄効果が硬性内視鏡に与える影響を比較し、有効性と効率を両立させたパラメーターの設定を決定すること。

方法

腹腔鏡下胆嚢摘出術に使われた硬性内視鏡600本を無作為に 4 群に分割した。1 つは、対照群（標準的なパラメーター）、3 つは、酵素洗浄時間、温度、または洗剤の濃度が異なる実験群であった。洗浄の質は、5× の拡大鏡やボアスコープによる目視検査、アデノシン三リン酸（ATP）生物発光検査、残留蛋白質の検出によって評価した。

結果

5× の拡大鏡またはボアスコープによる目視検査を使った洗浄適格率には、4 群間で有意差が見つからなかった（すべてP ＞ 0.05）。しかし、ATP 生物発光検査および残留蛋白質の検出では有意差が観察された（すべてP ＜ 0.05）。一対比較では、A 群は C、D 群よりも適格率が高かったことが明らかになった（P ＜ 0.0167）。ATP 生物発光検査において、群間で、相対発光量（RLU）の値に有意差が観察された（P ＜ 0.05）。すなわち、A 群は、B および C 群よりも RLU値が低く、D 群は、C 群よりも RLU 値が低かった（すべて P ＜ 0.001）。

結論

フラッシュおよびすすぎの段階の埋め合わせをするために真空沸騰洗浄消毒器の酵素洗浄の温度を高くすることで、硬性内視鏡の洗浄時間が短くなり、再処理の効率が向上した一方で、高い洗浄の質は維持された。
