小児における手術部位感染症予防の実施－横断的前向き研究★

背景
成人と小児の両方において、手術部位感染症（SSI）は罹患および死亡の一般的な原因である。小児外科では、特定の予防法に関するエビデンスはなく、診療は主に現地のガイドラインと治療チームの選好に依存している。
目的
標準化された調査ツールを用い、SSI 予防に関して、手術を受ける小児に対する現在の慣行を調査する。
方法
スイスの 9 カ所の小児外科センターが、標準的な期間の有病率研究に参加した。これらの病院において、2022 年 10 月の連続する 7 日間、手術創に至った小児外科手術に対して行われた SSI 予防法を記録した。注目する SSI 予防法は、世界保健機関の最新のガイドラインから抽出した。
結果
計 351 件の手術が対象となった。スイスのすべての言語地域が含まれていた。外傷／整形外科手術が最も多かった。351 件中 161 件（46％）に手術時に抗菌薬の予防的投与が行われていたが、161 件中 33 件（21％）は抗菌薬投与の適応がなかった。手術時の皮膚消毒には、アルコールベースまたはヨウ素ベースの溶液が最も多く使用されていた。抗菌薬をコーティングした縫合糸が使用されたのは 351 件中 84 件（24％）のみであった。皮膚消毒液、縫合糸の材料、創傷被覆材および加温装置の使用に関して、参加施設間で予防法に地域差が認められた。
結論
本研究により、スイスの小児外科センターにおける SSI 予防に関する現在の慣行の全体像が示され、改善のための目標が特定され、さらなる調査が必要な臨床的に不確実な領域が浮き彫りとなった。この結果は、小児外科において、一貫性のある、根拠に基づく SSI 予防戦略を保証するための標準化されたガイドラインの必要性を強調するものである。
