Silver-based dressings for the prevention of surgical site infections: evidence from randomized trials

背景

手術部位感染症（SSI）は術後合併症の主な原因であり、死亡率および医療コストを上昇させる。銀ベースのドレッシングは、抗菌作用があるため広く使用されているが、SSI の低減に対する有効性は依然として不明である。

目的

無作為化対照試験（RCT）における SSI 予防に対する有効性について、銀ベースドレッシングと非銀系ドレッシングを比較すること。

方法

PubMed、Cochrane Library および Embase を体系的に検索し、SSI 予防について銀ベースドレッシングと非銀系ドレッシングを比較した RCT を特定した。本研究のプロトコールは PROSPERO に登録した（CRD42024592966）。ランダム効果モデルを用いたメタアナリシスにおいて、統合リスク比（RR）および 95％信頼区間（CI）を算出した。不均一性を I² 統計量、出版バイアスをファンネルプロットおよび Egger 検定により評価した。

結果

12 件の RCT（参加者 2,928 例）を本メタアナリシスの対象とした。非銀系ドレッシングと比較して、銀ベースドレッシングにより SSI リスクは 40％低下し（RR 0.60、95％CI 0.41 ～ 0.89、P ＜ 0.05）、この差は有意であった。不均一性は中程度（I² ＝ 53％）であった。

結論

銀ベースドレッシングは有意に SSI のリスクを低下させるが、様々な手術状況および患者集団における有益性を確認するためには、質の高い RCT がさらに必要である。
