Emergence and establishment of Staphylococcus haemolyticus ST29 in two neonatal intensive care units in Western France

目的

2021 年以降、フランスでは、新生児集中治療室（NICU）におけるスタフィロコッカス・ヘモリティカス（Staphylococcus haemolyticus）のアウトブレイクが数件報告されている。この研究の目的は、S. haemolyticus が、新生児のケアを共有している 2 つの施設の NICU に、どのようにして定着したかを理解することであった。

方法

2020 年から2023 年の間に分離された S. haemolyticus 陽性の臨床検体すべてと、診療記録を解析した。表現型解析は、定量的アンチバイオグラム法を使ったタイピングと、全ゲノムシークエンシングを使った微生物学的調査から成っていた。環境検体と医療従事者の手からの検体を収集し、S. haemolyticus が検出された場合、同じ解析を実施した。医療従事者の衛生習慣についての観察研究も実施した。

結果

116 例の新生児が S. haemolyticus 陽性で、そのうち 44 例（38％）が感染していた。ST29 株が非常に多く、クローン集団を明確に区別できた。2 つの施設の間での新生児の転院とその後の交差伝播によって、1 つの集団での播種は説明できた。21 個の環境検体で、新生児に関与しているクローンの存在が明らかになった。検体を採取した医療従事者の 15％の手でも 1 つのクローン集団が見つかった。擦式アルコール製剤についての誤った考えが観察され、NICU の機器の毎日の消毒は最適ではなかった。

結論

予備的な報告では、NICU 内および NICU 間の交差伝播が指摘されている。それらは、医療従事者の手を介して直接、あるいは汚染された環境、特に保育器を介して間接的に起こる。ST29 系統は、大部分の NICU で検出されていて、その施設に順応しアウトブレイクを引き起こす能力がある。
