Meta-analysis of risk factors for infection by multi-drug-resistant organisms in intensive care unit patients

背景

多剤耐性菌(MDRO)による感染症患者は、合併症を発症することが多く、治癒が困難で、治療に最新の抗菌薬を必要とし、保菌状態になりやすい。このため、患者は重い経済的負担を負い、死亡に至ることもあり、一方で医療サービスにかかる経済的負担も増大する。

目的

本研究では、集中治療室(ICU)の患者における MDRO による感染症のリスク因子を系統的に評価した。

方法

PubMed、Embase、MEDLINE および Cochrane Library において、データベースの開始から 2023 年 9 月 21 日までの、ICU 患者における MDRO 感染症のリスク因子に関する文献を検索した。研究者 2 名が独立してスクリーニング、データ抽出、検索した論文の質の評価を行った。データの統計解析は Stata 16.0 を用いて行った。

結果

本メタアナリシスには 29 報の論文を組み入れた。対象となった患者は 18,063 例で、このうち 2,955 例が MDRO 感染症に罹患した。メタアナリシスの結果から、糖尿病、心血管疾患、過去 1年以内の入院歴、肝機能異常、MDRO 感染症の既往、外傷重症度スコア、ICU 滞在期間、経鼻胃管、経静脈栄養、保菌圧、多発外傷、機械的換気、気管切開、中心静脈カテーテル、抗菌薬投与歴、免疫抑制薬、ピペラシリン‐タゾバクタム、複数の抗菌薬投与、グリコペプチド系抗菌薬、カルバペネム、イミペネムおよびニトロイミダゾールが、ICU 患者における MDRO 感染症のリスク因子であることが明らかになった。

結論

ICU は MDRO 感染症の高リスク区域である。MDRO 感染症の発生を減少させるために、医療従事者はこれらのリスク因子に基づく予防・制御措置を講じるべきである。
