男性は女性より多剤耐性病原菌の保菌および感染リスクが高い

背景
多剤耐性菌(MDRO)の世界的な増加は警戒すべき水準にあり、抗菌薬耐性は世界的に重大な公衆衛生上の脅威となっている。最近の抗菌薬治療、抗菌薬の不適切な使用、入院などの特定のリスク因子が知られているが、MDRO の性別に特異的な側面に焦点を合わせたものは少ない。本研究の目的は、様々な MDRO の保菌および感染ならびにそれらの検出部位における性差を示すことであった。
方法
この多施設共同後向きコホート研究では、Helios Kliniken(ドイツ)の病院 86 施設から、2015 年から 2020 年までのサーベイランスデータを収集した。以下の多剤耐性菌を検体採取部位別に分析した:メチシリン耐性黄色ブドウ球菌(meticillin-resistant Staphylococcus aureus[MRSA])、腸球菌属菌(Enterococcus)、大腸菌(Escherichia coli)、肺炎桿菌(Klebsiella pneumoniae)、緑膿菌(Pseudomonas aeruginosa)およびアシネトバクター・バウマニー(Acinetobacter baumannii)。
結果
データベースに含まれた 7,081,708 例中、187,656 例(2.65%)で MDRO の保菌が認められた。MDRO 感染の記録は 33,023 例(0.466%)で特定され、24,231 例で感染源が判明していた。男性であることは、MDRO の感染および保菌の両方のリスク因子であった(P < 0.001)。男性では、皮膚表層/軟部組織、血液培養(P <0.001)および呼吸器検体(P = 0.002)で MDRO が検出される可能性がより高かった。さらに、MDRO 検出部位および病原体に性差が認められ、皮膚深層/軟部組織および呼吸器検体における MRSA 感染率は女性の方が若干高かった。
結論
本研究は、男性であることが MDRO の保菌および感染のリスク因子であるという従来の仮説を大規模データセットにより裏付け、補強するものである。これは、実臨床において性別に特異的な MDRO 感受性を認識する必要性を明らかにしている。
監訳者コメント:
疫学的に男性であることが MDRO の保菌および感染のリスク因子であることが本研究から導き出されているが、他の因子も分析をしてみる必要があろう。
