膝関節鏡視下手術を受けた患者における感染症のリスク因子:システマティックレビューおよびメタアナリシス

本研究の主要目的は、膝関節鏡視下手術後の感染症のリスクを特定し、手術部位感染症(SSI)のリスク因子を評価することであった。PubMed/Medline、Embase および Cochrane Libraryのデータベースを系統的に検索し、既報のシステマティックレビューおよびメタアナリシスの参考文献一覧を手動検索した。変量効果モデルを用いて統合オッズ比(OR)の推定を算出した。合計 38,321 報の候補論文が最初の選択基準に適合した。タイトル、アブストラクトおよびフルテキストを検討後、残った 41 報の論文を最終解析に組み入れた。論文 41 報において膝関節鏡視下手術を受けた患者 9,089,578 例が特定された。質の高いエビデンス(class I)では、自家移植片(十字靱帯再建術)(OR 2.66、95%信頼区間[CI]1.84 ~ 3.86)または手技の複雑度が高いこと(OR 2.02、95%CI 1.69 ~ 2.43)が、感染症のより高いリスクをもたらすことが示され、中等度の質のエビデンス(class II または class III)では、肥満(体格指数 ≧ 30 kg/m2)(OR 1.27、95%CI 1.08 ~ 1.49)、男性であること(OR 1.52、95%CI 1.32 ~ 1.75)、糖尿病(OR 1.71、95%CI 1.36 ~ 2.14)、喫煙(OR 1.65、95%CI 1.38 ~ 1.97)または術前のステロイド使用(OR 3.41、95%CI 2.10 ~ 5.54)が、感染症のより高いリスクをもたらすことが示された。メタアナリシスでは、年齢または抗菌薬予防投与と感染症発症との間に関連は認められなかった。このメタアナリシスにより、膝関節鏡視下手術後の感染症の有意なリスク因子は、肥満、男性であること、糖尿病、喫煙、手技の複雑度が高いこと、移植片の種類、術前のステロイド使用などであることが示された。

本研究では、膝関節鏡視下手術ではメタアナリシスにより膝関節鏡視下手術後の感染症の有意なリスク因子は肥満、男性であること、糖尿病、喫煙、手技の複雑度が高いこと、移植片の種類、術前のステロイド使用などが見いだされている。
