背景
フーリエ変換赤外(FT-IR)分光法は、ゲノムを使わないスペクトルに基づく型の決定技術で、多剤耐性微生物のアウトブレイクの特徴を明らかにするのに役立つ。
目的
院内アウトブレイクからの ST-80 バンコマイシン耐性エンテロコッカス・フェシウム(Enterococcus faecium)(VREf)分離株の特徴を明らかにすることにおける FT-IR 分光法の性能を評価すること。
方法
コアゲノム一塩基多型系統発生を参照法として使って、ST-80 VREf の引き起こした院内アウトブレイクの特徴を明らかにした。疫学的に関係のある 25 の分離株のうち、22 株が 1 つのアウトブレイク・クラスターに属していたことを特定した。
結果
FT-IR 分光法を、クラスターを決定するカットオフを 0.071 として使用したところ、1 つのクラスターに、系統発生的に関係のあった 22 の分離株のうち 21 株が正しく分類された。アウトブレイク・クラスターから、系統発生的に関係のなかった分離株 3 株、関係のない ST-80 の対照株 5 株、2023 年 5 月に起こった過去のアウトブレイクからの分離株 5 株を区別するのに成功した。誤って判定された環境分離株は 1 株のみであった。
結論
これらの結果は、アウトブレイクの調査を助ける迅速スクリーニングツールとして FT-IR 分光法が使用できる可能性を支持している。注目すべきは、この研究が、同じ配列型の VREf 分離株の疫学的解析において、FT-IR 分光法の性能に焦点を当てた最初の研究であることである。
