背景

クロストリジオイデス・ディフィシル(Clostridioides difficile)感染症(CDI)については、CDI のエピソードが 3 回以上ある患者に対して、現時点では、糞便細菌叢移植(FMT)が推奨されている。最近の無作為化比較試験(RCT)は、FMT を早期に検討してもよいことを示している。ここで、早期とは、CDI の 1 回目か 2 回目のエピソードでの介入と定義する。

目的

本研究の目的は、1 回目か 2 回目の CDI を起こしている患者で、補完治療としての FMT の入院費用を、現時点での標準治療と比較検討することであった。

結果

1 回目か 2 回目の CDI に対する標準治療と比べて、FMT に無作為化された患者の 26 週間に亘る入院費用は、入院や病院との連絡が少なく、薬剤の使用が少ないことから、1,645ユーロ低かった。感度分析でも、FMT による治療費が component 当たり約 1,572ユーロ未満に抑えられている限り、費用対効果は高いままであった。component 当たり 1,572ユーロは、FMT による治療(2 つの component)の総費用が 3,144ユーロ であることに相当した。

結論

FMT は、バンコマイシン単独療法を使う現時点での標準治療よりも、費用が安くなり、有効性が高くなるので、費用対効果が高かった。この結果は、感度分析でも強固で、2 つの component から成る FMT による治療 1 回の費用の閾値は 3,144ユーロであった。

監訳者コメント

日本では普及していないが、海外では Clostridioides difficile 感染症(CDI)の補完的治療としてカプセル化された糞便細菌叢移植(FMT)が行われている。本研究が行われたデンマークでは現時点で、CDI のエピソードが 3 回以上ある患者に対して推奨されている。本研究は CDI の早期(1 回目か 2 回目の CDI エピソード)での FMT の有用性を臨床的評価だけでなく、経済的評価の点からも示している。ただし、デンマークでは献血と似たような無償の献便制度があり、公的な FMT センターで管理させている点で、日本とは背景が異なる点には注意が必要である。
