Limited impact of colistin resistance on mortality of intensive care patients with carbapenem-resistant bacteraemia

背景

カルバペネム耐性グラム陰性菌(CR-GNB)菌血症の発生率の上昇により、ポリミキシンの使用が増加しており、コリスチン耐性の出現および拡大に拍車がかかる可能性がある。

目的

CR-GNB を有する集中治療患者において、コリスチン耐性の過剰な臨床的負担を推定した。

方法

4 年間(2020 年から 2023 年)にギリシャの大学病院の集中治療室(ICU)において、入院中に CR-GNB を発症した患者のコホートを構築した。競合リスク生存分析を行い、コリスチン耐性に関連する負担を推定した。

結果

CR-GNB を有するICU患者 177 例中、134 例(76%)からコリスチン耐性分離株が得られ、微量液体希釈法により、大部分がアシネトバクター・バウマニー(Acinetobacter baumannii)(79%)と同定された。コリスチン耐性感染症の患者において、年齢、性別、APACHE II スコア、Charlson 併存疾患指数スコア、Pitt 菌血症スコア、手術歴および複数菌培養の発生率は、コリスチン感受性感染症の患者と同様であった。しかし、コリスチン耐性群では、コリスチン感受性群と比較して、死亡リスクが低かった(14 日時点でそれぞれ 31% 対 44%、P = 0.004、28 日時点でそれぞれ 46% 対 56%、P = 0.173)。多変数回帰分析により、コリスチン耐性 CR-GNB は、コリスチン感受性 CR-GNB と比較して、感染症発現から 14 日以内(原因別ハザード比[csHR]0.53、95%CI 0.28 ~ 1.01)および 28 日以内(csHR 0.55、95%CI 0.31 ~ 0.95)の院内死亡リスクの有意な低下に関連することが確認された。

結論

CR-GNB を有する ICU 患者の大規模な同時コホートにおいて、死亡率に対するコリスチン耐性の限定的な影響が認められたが、これはおそらく、近年のコリスチンベース治療レジメンからの転換を反映している。

ギリシャはカルバペネム耐性グラム陰性菌(CR-GNB)の検出率が非常に高く、ポリミキシンの使用が増加することにより、コリスチン耐性菌の増加が問題になっている。近年はコリスチンによらない CR-GNB に対するセフタジジム/アビバクタム、セフトロザン/アビバクタム、イミペネム/レレバクタム、セフィデロールなどの新しい治療薬が導入されており、コリスチン耐性菌でも、これらの代替薬が使用されることから、患者の予後を悪化させなくなっているのであろう。
