Too much ado about data: continuous remote monitoring of water temperatures, circulation and throughput can assist in the reduction of hospital-associated waterborne infections

背景

国内および国際的なガイダンスは、医療建造物における給水システムの維持および管理に関する勧告を提示している。とはいえ、医療関連水系感染症は増加する傾向にある。

目的

医療建造物における水質にとって極めて重要なパラメーターを特定することと、遠隔センサーによる遠隔モニタリングが安全な給水システムを提供し、それによって医療関連水系感染症が低減するかどうかを評価すること。

方法

以下の検索用語を用いて記述的レビューを実施した：（1）「consistent water temperature（一定の水温）」AND「waterborne pathogen control（水系病原体制御）」OR「nosocomial infection（院内感染）」（2）「water throughput（水処理能力）」AND「waterborne pathogen control（水系病原体制御）」OR「nosocomial infection（院内感染）」（3）「remote monitoring of in-premises water systems（敷地内の給水システムの遠隔モニタリング）」AND「continuous surveillance for temperature（温度の継続的サーベイランス）」OR「throughput（処理能力）」OR「flow（流量）」OR「use（使用）」。使用したデータベースは、2023 年 1 月から 2024 年 3 月までの PubMed、CDSR（Clinical Study Data Request）、DARE（Database of Abstracts of Reviews of Effects）であった。

結果

バス・トイレ付きの患者用個室、手洗い洗面台の拡充、広範な手袋着用、アルコールゲル、ワイプが、給水システムの停滞を増大させており、水系病原体の増幅およびレジオネラ（legionella）、pseudomonas属、非結核性抗酸菌の伝播リスクもたらしている。手動モニタリングでは、大規模で複雑な給水システム全体にわたり温度が反映されない。本レビューにより、遠隔センサーによる多点連続モニタリングは、敷地内の給水システムにおいて、利用頻度の低い不要な排水口、水圧不均等性、ならびに一定しない温度送出を確認するという点で有効と考えられる。

結論

遠隔モニタリングが一般的になるにつれ、温度制御、水圧、給水システムの均等性の不具合に関する認識も高まるであろう。そして、当病院内で開発中の本技術を導入する際に、学ぶべきことは多くある。

監訳者コメント：

病院の給水システムによって医療関連水系感染症が媒介され伝播する可能性があり、そのリスク低減のために定期的なモニタリングを行うことは有用である。
