水を使用しない患者ケア：今後の指針に向けた緊急の課題に関する文献および議論の記述的レビュー★

背景
フローレンス・ナイチンゲールは、構築環境と患者の健康障害との関連を認識した最初の人である。それから 160 年を経た今、抗菌薬時代の終わりの脅威が大きく立ちはだかっている。抗菌薬耐性対策アクションプランは、抗菌薬適正使用支援および新規治療薬の開発に焦点を置いている。構築環境のリスクは無視されており、医療施設内の抗菌薬耐性の主な原因として廃水システムが特定されている。英国は、最大の医療構築プログラムに包括的に着手している。このような施設は、抗菌薬耐性がもっとも深刻な状態であると予測される場合に機能することになる。水を使用しない患者ケアは、抗菌薬耐性の拡散の制御、ならびに新たな病院においてさらなる評価を必要とする患者の感染症の予防を図るための対策である。
方法
以下の用語を用いて記述的レビューを実施した：「waterless/water-free unit（水を使用しない／水を使用しない病棟）」、「waterless/water-free care（水を使用しない／水を使用しないケア）」、「sink reduction（シンク削減）」、「sink removal（シンク除去）」「washing without water（水を使用しない洗浄）」。PubMed、Cochrane Database of Systematic Reviews、Database of Abstracts of Reviews of Effects を用いて、総説および原著論文を 2000 年 1 月から 2024 年 2 月まで検索した。病棟の種類、地理的地域、水を使用しないアプローチの理由、ならびにアウトカムについて記録した。
結果
7 報の論文が特定された。4 報は成人集中治療室（ICU）、1 報は高齢者ケア施設、2 報は新生児 ICU を対象とした。5 報における介入の目的は、グラム陰性菌感染症／保菌を低減することであった。1 報は、費用対効果および患者の体験を調査した「水を使用しない洗浄」に関するシステマティックレビューであった。グラム陰性菌に焦点を置いた 5 報の論文はすべて、介入後の感染症または保菌の低減を報告した。
結論
水および廃水が患者の安全性に及ぼすリスク、ならびに感染率低減のために水を使用しない対策の重要性を強調する研究がより多かった。
監訳者コメント ：
医療環境で水を介した医療関連感染がどのように発生しているのか、過去の論文を検証してみた論文である。このような分析を行うことで、医療関連感染の原因特定の糸口がわかる。
