清掃の時間と動き:病院で共有される医療機器を効果的に清掃するのに必要な時間についての観察研究★

背景
医療関連感染のリスクの減少において清掃は重要な役割を果たしているにもかかわらず、病院でよく使われる共用の医療機器の様々な部品の効果的な清掃と消毒に必要な時間を定量する研究は企てられていない。この短報では、共用の医療機器のありふれた部品の効果的な清掃に必要な時間の定量を目的とした研究の結果を提示する。
方法
病院でよく使われる共用の医療機器の 12 個の部品の効果的な清掃と消毒に必要な時間を明らかにするために、看護シミュレーション実験室で、時間と動きの観察研究を実施した。訓練の後、参加者は、機器の清掃と消毒を行い、清掃にかかった時間を記録した。清掃の過程で紫外線蛍光点の 80%以上が取り除かれた場合に、清掃は効果的であったと考えた。
主な結果
機器を効果的に清掃するための時間は、50 秒(血糖検査キット、95%信頼区間[CI]、40 秒 ~ 1 分)から 3 分 53 秒(薬用カート、95%CI、3 分 36 秒 ~ 4分11秒)までの範囲であった。最も効果的に清掃できたのは、点滴スタンドで、点が 100%取り除けた(N = 100 個の点)。それとは対照的に、清掃が最も難しかったのは、膀胱スキャナーで、効果的な清掃とした 80%の閾値に達するまでに 12 回試みることが必要であった。
結論
この研究は、共用の医療機器の効果的な清掃と消毒を計画する際に必要な職員の配置と訓練についての情報を提供するだろう。結果は、ビジネスケースの作成や、共用の医療機器を含む清掃による介入の費用対効果を将来評価する際にも使うことができる。
監訳者コメント :
医療現場で共用の医療機器が多く使用されているが、医療機器ごとに清掃しやすさは異なっていた。複雑な構造の医療機器の清掃訓練に取り組む必要があるだろう。
