背景

緑膿菌(Pseudomonas aeruginosa)は院内グラム陰性菌菌血症の最大の原因である。給水システムが緑膿菌の発生源であることがよく知られており、定着したバイオフィルムは除去が困難である。

目的

細菌定着蛇口モデルを用いて、定期的なフラッシュ洗浄、過酢酸消毒、蛇口内の熱消毒および配管内の熱消毒の緑膿菌バイオフィルム除去における有効性を評価した。

方法

模擬蛇口システムを作成し、緑膿菌の基準株および環境株を播種してバイオフィルムを形成させた。蛇口から水サンプルを採取し、消毒法実施後の緑膿菌数を計数した。定期的なフラッシュ洗浄をシミュレーションするために、1 日 5 回、5 分間蛇口から水を流し、水を毎日検査した。過酢酸(4,000 ppm)を手動でシステムに注入し、ポンプでシステム全体に流した。配管内で、および蛇口内のバイパス弁を用いて、60°C で熱洗浄を行った。架橋ポリエチレン配管で検査を行い、銅配管で再度検査を行った。

結果

定期的なフラッシュ洗浄およびポンプを用いた過酢酸注入では、緑膿菌数は減少しなかった。過酢酸を手動で注入した場合に、限定的な減少が認められた。蛇口内の熱洗浄により、銅配管では緑膿菌が除去されたが、架橋ポリエチレンでは除去されなかった。配管内の熱洗浄は、緑膿菌数を減少させる効果が最も高かったが、バイオフィルムの除去には至らなかった。

結論

配管内の熱洗浄が、緑膿菌バイオフィルムを除去するための最も効果的な方法であった。結果は、菌株および配管の構成によって大きく異なる。定着したバイオフィルムを除去するためには、いくつかの方法を併用する必要があると考えられる。

監訳者コメント

緑膿菌は『緑膿菌バイオフィルム』に対抗する手段が実務的に必要となる。菌自体を死滅することができても、感染予防にはその菌を守っている鎧(つまり緑膿菌のバイオフィルム)を破壊しなければならない。
