Assessing morinidazole for surgical site infection in class III wounds prevention: a multi-centre, randomized, single-blind, parallel-controlled study

緒言

手術部位感染症(SSI)は重大な術後リスクであり、嫌気性菌の存在により予防的抗菌薬投与が不可欠である。クラス III 手術創の SSI 低減を図るための新規ニトロイミダゾールであるモリニダゾールについて、今のところ既報でのエビデンスが不足しているので、本研究ではその有効性および安全性を検討した。

方法

中国の 3 次病院 12 施設の一般外科において、多施設共同無作為化臨床試験を 2020 年 12 月から 2022 年 10 月まで実施した。患者 459 例を対象とし、モリニダゾールとセフトリアキソン併用、またはセフトリアキソン単独による 2 つの治療群に割り付けた。有効性および安全性の評価には、SSI 発生率、有害事象、合併症などを含めた。統計解析には SAS 9.4 ソフトウェアを用いた。2023 年 2 月から 5 月までデータの解析を実施した。

結果

参加者 440 例(中央値[四分位範囲:IQR])、年齢 63.0 歳(54.0 歳から 70.0 歳)、男性 282 例(64.09%)、漢民族 437 例(99.32%)で、参加者を無作為化した。SSI 発生率は、試験群のほうが対照群と比較して有意に低かった(31 例[14.49%]対 52 例[23.01%]、リスク差 1.76%、95%信頼区間[CI]1.08 ~ 2.88%、P = 0.0224)。表層切開部感染症は、試験群で顕著な減少が認められた(12 例[5.61%]対 31 例[13.37%]、リスク差 2.68%、95%CI 1.34 ~ 5.36%、P = 0.0042)。手術部位以外の感染症、重篤な術後合併症、全有害事象について統計学的に有意な群間差は認められなかった(P > 0.05)。

結論

SSI 発生率および表層切開部感染症の有意な減少は、モリニダゾールが有用な予防的抗菌薬であることを証明するものである。本研究の結果は、臨床における有益な洞察をもたらし、この次世代ニトロイミダゾールは SSI 予防に重要な役割を担うと考えられる。
