resistance か tolerance か？消毒分野における正確な専門用語の必要性を重視

「resistance」「tolerance」の専門用語は、抗菌薬研究の領域では明確に定義されている。しかし、消毒分野では、これらの用語は同義語として使用されることが多く、曖昧さが生じ、誤解や誤認を招く可能性がある。さらに、このような専門用語の不一致は、消毒薬耐性（resistance）のリスク評価を困難にする。本総論の目的は、消毒薬の観点から「適合」「感受性」「tolerance」「持続」「resistance」の用語の既存の定義について検討することである。tolerance と resistance においてもっとも曖昧さがみられる。tolerance は、通常での致死濃度への一過性曝露に対して必ずしも遺伝的ではない生存能力を示すのに対して、resistance は、曝露時間にかかわらず、それ以外で抗菌薬の致死濃度に耐える、厳密には遺伝的能力である。抗生物質および消毒薬はいずれも抗菌薬であるが、両者には重要な違いがあるため、消毒薬耐性（resistance）のリスクを評価する場合、抗生物質の研究から得た経験の単なる転用は推奨されない：（i）消毒薬は通常、最小発育阻止濃度を桁違いに超える濃度で適用される、（ii）消毒薬の曝露時間は、数秒、数分、または数時間の範囲内である、（iii）消毒薬の作用機序の特異性は低い、（iv）消毒薬は、相加効果または相乗効果を示す 2 種類以上の作用成分を含むことが多い。抗生物質など他の抗菌薬と同様に、消毒薬は二つの性質を有しており、正しく慎重に使用すべきであるという認識が重要である。
監訳者コメント：
専門用語の意味づけを明確にしないと、科学的（医学的）な説明ができないことになる。この点はくれぐれも強調したい。
