Effect of topical vancomycin powder on surgical site infection prevention in major orthopaedic surgery: a systematic review and meta-analysis of randomized controlled trials with trial sequential analysis

背景

手術部位感染症（SSI）の低減におけるバンコマイシンパウダー局所投与の効果に関してエビデンスは混在している。

目的

整形外科大手術において SSI 予防のためのバンコマイシンパウダー局所投与の効果を明らかにすること。

方法

MEDLINE、Embase、CENTRAL、ICTRP、ClinicalTrials.gov のデータベースを用いて、発端から 2023 年 9 月 25 日まで検索した。整形外科大手術において SSI 予防を図るためのバンコマイシンパウダー局所投与と対照を比較する無作為化対照試験を含めた。2 名の評価者が独立してタイトルと抄録のスクリーニングならびに関連するデータの抽出を行い、次いでバイアスリスクとエビデンスの確実性の評価を実施した。主要評価項目は、全 SSI、再手術、有害事象とした。ランダム効果メタアナリシスを用いて要約の結果を得た。試験逐次解析を実施した。

結果

無作為化対照試験 8 件から参加者 4,307 例のデータを入手した。対照と比較して、バンコマイシンパウダーは、全 SSI の低減に差を示さなかった。患者の累積数は試験逐次解析における必要情報量 19,233 例を超えず、Z 曲線は試験逐次監視境界も無益性境界も超えなかったため、メタアナリシスの結果は決定的ではないことが示唆された。再手術について差は認められなかった。SSI の中でもグラム陽性球菌による SSI の低減にバンコマイシンパウダーは統計学的有意差を示した。しかし、このエビデンスの確実性はきわめて低かった。

結論

このシステマティックレビューとメタアナリシスでは、整形外科大手術における SSI 低減にバンコマイシンパウダーが及ぼす効果に関して、結果は決定的ではないことが示唆される。この介入の効果を明らかにするために厳密な方法を用いたさらなる試験が必要である。
