Logistic regression model predicts early surgical site infection after spinal fusion: a retrospective cohort study

目的

この研究は、脊椎固定術後早期の手術部位感染症(SSI)を予測するための術後の炎症マーカーに基づいた診断モデルを開発することを目的とした。

方法

この後向き研究で、我々は、炎症マーカーの傾向を SSI 群と非 SSI 群の間で解析した。データは、訓練コホートと検証コホートに無作為に分けた(比 7:3)。SSI についての変数を段階的ロジスティック回帰により解析し、予測モデルを開発した。モデルを評価するために、その感度、特異度、陽性的中率、陰性的中率、曲線下面積を検証コホートで解析した。キャリブレーションプロットと決定曲線分析を使って、モデルのキャリブレーションと臨床的有用性を評価した。

結果

手術後 7 日目に、炎症マーカーの有意な変化を観察した。予測モデルには、手術後 7 日目における 4 つの変数を組み入れた。すなわち、体温、C 反応性蛋白質、赤血球沈降速度、好中球数である。これらのデータの 2 値処理の後、単純化されたモデルは、訓練コホートにおいて、曲線下面積が 0.86 (95%信頼区間[CI]、0.81 ~ 0.92)を達成し、検証コホートにおいて、0.9(95%CI、0.82 ~ 0.98)を達成した。キャリブレーションプロットと決定曲線分析によって、提案したモデルが SSI の診断に有効であったことが示された。

結論

我々は、脊椎固定術後早期の感染を診断する予測モデルを開発し、妥当性を確認した。

監訳者コメント

脊椎固定術後の感染率は 0.7%~ 16%と幅があり、発生するとインプラント抜去、固定失敗、時に生命予後に影響するような重篤な SSI となることがある。その結果、入院期間の延長、医療費の増大に加え、患者の QOL を著しく低下させるため、早期診断・早期治療は重要である。近年では AI(人工知能)を使った SSI予測モデルもある。一方、SSI の診断には培養検査が必須であり、結果判明に時間を要するため、早期診断には培養以外の方法が必要となり、本論文のような予測モデルが作成された。しかしながら、後方視的研究であり、症例数も少ないため、より良い予測モデルを作成するためには、もっと大きなサンプル数で前向きな多施設研究が必要である。
