背景

臨床病原体の伝播における医療環境の役割は確立されている。欧州規格(EN)17126:2018 は、統制された殺芽胞製剤試験の必要性に対応するために開発されたものであり、現実的な医療汚染物質を含み、洗浄と消毒の複合効果を標榜する製剤を検証できるようになっている。

目的

模擬的な清潔条件および医療汚染条件の存在下で、酸化消毒薬製剤の化学的安定性と殺芽胞効果を調べた。

方法

消毒薬の安定性および殺芽胞効果は、同一条件下での汚染物質:製剤の比で評価した。消毒薬を試験用模擬汚染物質に曝露し、標準比色法を用いて遊離塩素、二酸化塩素、過酢酸の濃度を測定した。C. difficile R027 内生胞子に対する消毒薬の効果を、EN 17126:2018 に従って評価した。清潔条件と医療汚染条件との性能の比較は、一元配置分散分析を用いて行った。Pearson 積率相関を用いて相関分析を行った。

結果

塩素放出剤(ジクロロイソシアヌル酸ナトリウム、二酸化塩素、次亜塩素酸)の性能は濃度に依存し、1,000 ppm の塩素は汚染条件下で安定性および効果が低下した。一方、過酢酸製剤は、汚染条件下でも安定性を示し、一貫して効果を達成した。

結論

効果のない環境汚染除去は、医療関連感染症を引き起こしうる病原体の伝播リスクを上昇させる可能性があることから、これらの結果は実臨床に影響を与える。

監訳者コメント:

汚染のある環境では塩素系消毒薬の有効性は低下したが、過酢酸製剤の有効性には低下がみられなかった。消毒薬の効果を得るためには汚染除去が重要であること、そして仮に汚染があった場合に消毒薬によって有効性が異なることが示された。
