Clinical challenge of diagnosing non-ventilator hospital acquired pneumonia and identifying causative pathogens: a narrative review

非人工呼吸器病院感染肺炎（NV-HAP）は大きな医療負担と関連しており、これは高罹患率と、それに伴う病的状態および死亡に起因する。しかし、症例の正確な特定は依然として困難である。現時点では、NV-HAP 診断においてゴールドスタンダードとなる検査はなく、その代わりに非特異的徴候と検査を組み合わせる必要がある。原因菌はごく少数の症例で特定されるにとどまっている。これは、サーベイランス、患者の転帰、抗菌薬適正使用支援において大きな影響を与えている。HAP の既存の研究の多くは、人工呼吸器患者において実施されている。NV-HAP に特化した研究が不足しているため、診断方法、病理、介入に関する結論は、他の設定における研究をもとに、かなり推定せざるを得ない。NV-HAP の広く同意された定義がないことで進展も制限される。NV-HAP の診断は改善の余地が大きい。症例定義に関してコンセンサスが得られると、有意義な調査によって、患者が経験する転帰の原因やばらつきについて理解を向上させることができる。画像診断の役割を最大限に利用でき、原因と思われる病原体を同定するための新たな技術を組み込める可能性がある。これにより重大な医療関連感染症の抗菌薬適正使用支援とサーベイランスの両方が推進されるであろう。本記述的レビューは、既存の NV-HAP 診断法の有用性について、病原体同定の重要性および課題に焦点を置き考察する。また、現行の技術の限界を検討し、微生物学的診断および患者の転帰の改善を図るための新たな分子技術の可能性を探求する。
