英国の産科における PVL 陽性 MRSA アウトブレイクの発生および制御★

本稿の目的は、英国の産科と新生児科において、全ゲノムシークエンシング（WGS）により確認した USA300 ST8 PantoneValentine leucocidin（PVL）陽性メチシリン耐性黄色ブドウ球菌（meticillin-resistant Staphylococcus aureus：MRSA）アウトブレイクの調査および制御について報告することである。2 例の関連する PVL-MRSA 症例が特定されたため、アウトブレイクの調査を実施した。感染制御サーベイランスデータベースを用いた遡及調査、保存された MRSA 分離株のタイピング、患者のスクリーニングの強化および職員のスクリーニングを用いて追加の症例を特定した。環境スクリーニングも実施した。分離株間の遺伝的関連を、WGS を用いて評価した。アウトブレイク中の 2021 年 7 月 11 日から 2022 年 12 月 22 日までに 18 例が特定され、このうち 10 例は感染、8 例は保菌であった。医療従事者 1 例が同一株の保菌陽性を示し、スワブを用いた環境表面からのサンプル採取により、院内の情報技術機器の汚染が検出された。アウトブレイクは、保菌が認められた医療従事者を出勤停止とし、感染予防・制御策を講じたことにより終息した。アウトブレイクの終息以降に、類似する分子プロファイルを有する PVL-MRSA が市中で検出されている。医療従事者が PVL-MRSA の伝播に関与した可能性がある。医療従事者を出勤停止とし、除菌を実施したことが、さらなる症例の発生を防ぐ要であった。WGS は、症例の特定および関連付けにおいて有用であった。市中の類似する分子プロファイルを有する PVL-MRSA 症例を特定することにより、医療環境外への本菌の伝播を確認できる。
