背景

重症熱傷患者は抗菌薬耐性菌による重症感染症をきたしやすく、カルバペネム耐性腸内細菌目細菌(CRE)獲得のリスク因子を必然的にいくつか有しているが、こうした患者における CRE 感染症または保菌のリスク因子は検討されていない。

目的

重症熱傷患者における CRE 獲得の独立リスク因子を特定すること。

方法

急性熱傷治療のために熱傷集中治療室に入室した患者を、熱傷集中治療時の培養結果に基づいて、CRE グループと非 CRE グループに分類し、後者にはカルバペネム感受性腸内細菌目細菌(CSE)グループと対照グループが含まれた。臨床的因子および微生物学的因子について、CRE グループと非 CRE グループの間で、また CRE グループと CSE グループの間で比較して、院内 CRE 獲得の独立リスク因子の特定を試みた。

結果

対象とした患者 489 例のうち、101 例(20.7%)が CRE グループ、388 例(79.3%)が非 CRE グループに分類された。非 CRE グループでは、91 例(18.6%)が CSE グループ、297 例(60.7%)が対照グループに分類された。CRE グループと非 CRE グループを比較する多変量解析では、他の CRE 獲得患者への曝露(P = 0.018)、abbreviated burn severity index スコア≧9(P = 0.012)、および機械的換気(P < 0.001)が CRE 獲得と関連していた。CRE グループと CSE グループを比較する多変量解析では、他の CRE 獲得患者への曝露が CRE 獲得と関連していた(P = 0.048)。

結論

入院患者における熱傷の重症度のコントロールに限界があることを考慮すれば、重症熱傷患者における院内 CRE 伝播を予防する感染制御策の強化を重要視すべきである。
