癌患者における院内血流感染症：現在の知識および今後の方向性

癌患者では、院内の一般集団と比較して、院内血流感染症（BSI）および中心ライン関連血流感染症（CLABSI）による予防可能な障害の発生率が高い。癌患者における院内 BSI および CLABSI の予防は緊急の優先事項であり、標準化サーベイランスと報告のための取り組みが必要とされる。
癌患者における院内 BSI および CLABSI の定義、分類システム、およびサーベイランス対策の適用は難しく、臨床診療において大きなばらつきがある。既存のシステムは、癌患者向けに明確に設定されておらず、癌の背景においてメリットとデメリットが異なる。このような理由で、癌患者の院内 BSI および CLABSI の疫学的な推定には、慎重な解釈も必要となる。この複雑さは、介入の適切な対象を決定したり、予防可能な障害を低減したりする際の障壁となりうる。
本総説では、癌患者に特化した院内 BSI サーベイランスおよび予防に関して、重要な概念と課題の全体像を提示する。さらに、本総説は、癌治療、現在のサーベイランス実施、院内 BSI および CLABSI の疫学、予防策、ならびに最新の知識のギャップを背景に、よく使用されるサーベイランスの定義および共通項のメリットとデメリットの概要を示す。 癌患者における院内感染症のサーベイランスを調整するための国際共同での包括的な取り組みは、既存データの精度と有用性を高め、院内感染症の予防活動を推進し、患者の安全性を改善する上できわめて有益であろう。

入院中の癌患者における、院内 BSI および CLABSI の総説である。癌の治療の上で、輸液療法は欠かすことができない。その最中に発生しうる院内 BSI および CLABSI は注意する必要がある。
