緑膿菌(Pseudomonas aeruginosa)菌血症に対する短期治療と長期治療の比較:システマティックレビューおよびメタアナリシス★★

緑膿菌(Pseudomonas aeruginosa)血流感染症に対する最適な治療期間は知られておらず、感染症の重症度、患者における病態の複雑さ、多剤耐性のリスク、ならびに高い死亡率のために、長期治療の方が好まれることが多い。そこで我々は、緑膿菌血流感染症に対する短期治療と長期治療を直接比較した研究を対象に、システマティックレビューおよびメタアナリシスを実施した。包括的検索を、Ovid MEDLINE、Embase、Cochrane Central Register of Controlled Trials、Cochrane Database of Systematic Reviews、および Scopus などで実施した。我々は、DerSimoniane-Laird ランダム効果モデルを用いてリスク比をプールし、全死因死亡、再発性感染症、ならびにこれらの転帰の複合について、サブグループ解析を実施して、緑膿菌血流感染症に対して短期治療と長期治療を受けた患者を比較した。不均一性を、I2 指標を用いて評価した。コホート研究におけるバイアスのリスクを、ROBINS-I ツールを用いて評価した。特定された研究 908 件中 6 件をシステマティックレビューの対象とし、治療期間について直接比較を行った研究 5 件をメタアナリシスで評価し、患者数は合計で 1,746 例となった。プロペンシティスコアで重み付けした複合アウトカム(30 日全死因死亡または再発性感染症)には、短期治療または長期治療を受けた患者の間で有意差は認められず、プールしたリスク比は 0.80(95%信頼区間 0.51 ~ 1.25、P = 0.32、I2 = 0%)であった。さらに、治療期間は、30 日全死因死亡、または再発性/持続性感染症という個々のアウトカムに対して影響を及ぼさなかった。我々のメタアナリシスでは、緑膿菌血流感染症に対して、短期間の抗菌薬療法は長期治療と同様の効果がある可能性が示された。緑膿菌菌血症に対する最適な管理を決定的に明らかにするには、将来的にランダム化試験が必要となろう。
監訳者コメント:
システマティックレビュー・メタアナリシスにより、緑膿菌血流感染症に対して短期治療あるいは長期治療を受けた患者の 30 日全死因死亡率と再発率は同様であった。
