ルーチンの患者ケアに自動電子化手指衛生モニタリングシステムを導入すべきか？ 複雑介入のための Medical Research Council フレームワークを用いたシステマティックレビューおよび評価★★

手動による手指衛生監視は、時間と多大な労働力を要し、しかも不正確である。自動手指衛生モニタリングシステムには利点がある（標準化データの作成、ホーソン効果の回避）。2009 年に発表された世界保健機関の手指衛生ガイドラインは、自動手指衛生モニタリングシステムが代替手段となる可能性を示している。本レビューの目的は、自動手指衛生モニタリングシステムに関する文献の現状を評価することと、実際の環境での使用に向けて推奨事項を提示することである。本稿は文献のシステマティックレビューであり、PubMed の開始時から 2023 年 11 月 19 日までの公表文献を対象とした。43 の公表文献が基準を満たした。複雑介入の開発および評価のための Medical Research Council フレームワークを用いて、2 報は介入開発研究に、39 報は評価に分類された。2 報は実際の環境における実装について報告した。小規模で短期間の研究が大部分を占めた。自動手指衛生モニタリングシステムと追加介入（視覚的または聴覚的指示、実践のフィードバック）を併用することで、短期間に手指衛生遵守の向上が可能となった。感染率への影響を明らかにするのは困難であった。費用および資源について検討した少数の公表文献によると、自動手指衛生モニタリングシステムが使用されている場合、手指衛生遵守の改善に充てる時間が増加した。自動手指衛生モニタリングシステムに関する医療従事者の意見は混在していた。結論として、現時点では、自動手指衛生モニタリングシステムの長期的な利益についてはほとんど知られていないので、ルーチンの患者ケアへの導入を推奨できない。より長期的な実装の評価（12 か月超）が得られるまでは、手指衛生遵守の直接観察の改善に向けて取り組み、その正確度と信頼性を改善すべきである。Medical Research Council フレームワークは、感染予防技術の使用など他の複雑介入を分類するために使用可能であり、実装に対して準備を確立するのに有用である。
監訳者コメント：
自動手指衛生モニタリングシステムは、実装することによって個々の医療従事者のモニタリングとリアルタイムに近い手指衛生の改善が見込まれる。日々、こうした改善計画により医療従事者のルーチンな手指衛生も徹底できるに違いない。
