背景

本研究では、脊椎手術後の術後手術部位感染症(SSI)を予測する機械学習(ML)アルゴリズムに組み入れるリスク因子を検討した。

方法

本前向きコホート研究は、2015 年 1 月から 2022 年 10 月までに南京医科大学附属 Taizhou People’s Hospital で脊椎手術を受けた患者 986 例を対象とした。教師あり ML アルゴリズムとして、サポートベクターマシン、ロジスティック回帰、ランダムフォレスト、XGboost、決定木、k-最近傍法、ナイーブベイズ(NB)を試験および訓練し、予測モデルを構築した。ML モデルの性能は、試験データセットから評価した。これらのモデルの正解率、感度および特異度、さらに陽性的中率、陰性的中率および曲線下面積を、逐次的に解析した。

結果

SSI 発生率は 9.33%であった。後方ステップワイズ法を用い、多変量 Cox 回帰分析において、SSI を予測する注目すべきリスク因子は年齢、体格指数、喫煙、脳脊髄液漏出、ドレーン留置期間および術前のアルブミン値であることがわかった。他の ML アルゴリズムと比較して、NB モデルは 7 つの ML モデルの中で最も性能が高く、平均曲線下面積 0.95、感度 0.78、特異度 0.88、正解率 0.87 であった。

結論

ML アルゴリズムの中で NB モデルは、既存モデルと比較して優れた較正能を示し、SSI のリスクを正確に予測したことから、脊椎手術後の SSI の早期発見および治療のための重要なツールとなる可能性がある。

監訳者コメント:

感染対策の領域は、AI との相性が良く、最近急速に論文が増えてきた。AI の予測においては、「なぜそう予測したのか」理由が分からないものも多いが、本論文は、リスク因子を明示してくれている。ただそれにしても「測定(想定)していないパラメーター」は検討できないのは相変わらずであり、今後は患者情報収集の点でも更なる発展が期待される。
