背景

カルバペネマーゼ産生腸内細菌目細菌(CPE)は罹患率と死亡率が高いために、世界中で深刻に懸念されている。

目的

監視培養が連続で陰性となった後に行ったカルバペネマーゼについてのポリメラーゼ連鎖反応(PCR)検査の結果が、CPE の監視培養が 3 回連続で陰性であった患者の培養が陽性転化することに対して与える影響を評価することと、転化のリスク因子を特定すること。

方法

CHROMagar KPC 培地での CPE の培養が陽性であった患者の後ろ向き研究を、韓国の 3 次病院で 2018 年 10 月から2022 年 12 月まで実施した。直腸スワブの培養が 3 回連続で陰性となった後、blaKPCblaNDMblaIMPblaVIMblaGESblaOXA-48 の PCR を実施した。患者の臨床特性および転帰を、追跡調査の PCR が陽性か陰性かに従って比較した。

結果

CPE の培養が陽性であった1,075 例の患者のうち、150 例(14.0%)で、直腸スワブの培養が 3 回連続で陰性となった。これらのうち、50 例(33.3%)は、追跡調査の PCR が陽性で、100 例(66.7%)は、陰性であった。多変量解析で、PCR の結果が陽性であることと関連していたリスク因子は、年齢、中心静脈カテーテル、大腸菌(Escherichia coli)の感染であった。追跡調査の PCR が陽性であった患者は、陰性であった患者より、CPEの培養が陽性転化する可能性が高かった(39/44[88.6%]対 21/50[42.0%]、P < 0.001)。多変量解析では、培養の転化の独立したリスク因子は、監視培養後の PCR の結果が陽性であったこと、糖尿病、中心静脈カテーテル、肺炎桿菌(Klebsiella pneumoniae)であった。

結論

追跡調査の PCR が陽性であった患者は、陰性であった患者より、培養の転化率が高かった。また、監視培養が連続で陰性となった後の追跡調査の PCR 検査は、接触予防策を中止するかどうかを決めるのに役立つ。

監訳者コメント

薬剤耐性菌に対する接触予防策の解除の基準に明確なものははなく、例えば培養 2 回ないしは 3 回陰性で解除する施設もあれば、中止の基準は設けずに接触予防策を継続する施設もあり、さまざまな運用がなされているのが現状である。培養のみならず、本研究のように耐性遺伝子の PCR 検査を組み込むことによって、安全かつ早期に対策の解除を行うことができる可能性がある。
