緒言

免疫不全患者は、重症レジオネラ感染症の発症リスクが高い。本稿では、トイレ貯水タンクからの汚染水に関連した院内感染レジオネラ症の死亡例を受けて開始された、アウトブレイク調査の結果を示す。さらに、貯水タンクにおけるレジオネラ属菌の増殖に関する実験データを提示し、簡単な予防プロトコールを提案する。

方法

建物の温水および冷水システムにおけるレジオネラ属菌の増殖を、選択的寒天培地での定量的細菌培養を用いてモニターした。感染患者および給水システムから得たレジオネラ・ニューモフィラ(Legionella pneumophila)分離株の分子タイピングを、コアゲノム複数部位塩基配列タイピング(cgMLST)により実施した。

結果

レジオネラ汚染度は、病院建物の冷水システムにおいて温水システムより有意に高く、トイレ貯水タンクの水において、バスルームのシンクおよびシャワーの冷水と比較して有意に高かった。患者から得た分離株と患者のバスルームの貯水タンクから得た分離株が、cgMLSTにおいて一致した。実験的環境では、21 日間毎日トイレの水を流すことにより、トイレ貯水タンクの水におけるレジオネラ属菌の増殖が 67%減少した。さらに、過酢酸により貯水タンクを 1 回消毒した後、毎日水を流すことにより、これらの環境で少なくとも 7 週間にわたって、レジオネラの増殖が 1%未満に減少した。

結論

短期的手段として、高度に汚染された貯水タンクを過酢酸で 1 回消毒し、毎日トイレの水を流すことにより、貯水タンク内のレジオネラ汚染を有意に減少させることが可能である。これらの手段は、免疫不全患者におけるレジオネラ感染の予防に役立つ可能性がある。

監訳者コメント

病院環境におけるレジオネラの生息場所は、冷却塔や浴槽、加湿器や吸入器などが知られているが、トイレの貯水タンクにも存在した、という論文。最近の日本の検討でもトイレ貯水タンクからレジオネラが検出されており、レジオネラ対策の難しさを改めて実感させられる論文である。
