ノルウェーのナーシングホーム 2 施設において医療従事者はどのようにして、またどのような理由で手袋を使用するのか？★

背景
手袋の不正確な使用は、交差汚染および医療関連感染症の原因となり得る。これまでの研究により、このようになる理由が特定されており、例として適応がないこと、不適切な交換、および手指衛生ポストの不十分な使用などが挙げられる。医療従事者の手袋使用の背景にある理由を調べた研究は限られている。
目的
手袋の使用に関する医療従事者の態度および認識について詳細な理解を得ること、また障害と促進因子、ならびに国内ガイドラインの遵守状況を調べること。
方法
小規模の収束並列混合法研究デザインを用い、これには観察と、Systems Engineering Initiative for Patient Safety model（SEIPS）を用いたフォーカスグループ面談を併用した。本研究は、ノルウェーのオスロにある市立ナーシングホーム 2 施設において、2023 年 1 月から 2 月の 2 週間にわたり実施した。
結果
観察 73 件中 67 件では手袋が用いられ、観察 6 件では適応があっても手袋が着用されず、使い回しが 16 件（21.9％）観察された。手袋を脱いだ後の手指衛生の不実施が、67 件中 36 件（53.7％）で観察された。2回のフォーカスグループ面談から、手袋使用の決定は、習慣、知識、経験、および患者の要求と結び付いた感情、業務、および手袋の種類に基づいていたことが示された。二重手袋の使用は、自身の保護および効率性が理由であった。手袋の利用可能性が促進因子であり、手袋の質の不良が障害であった。
結論
医療従事者が手袋使用の指針を知っているにもかかわらず、その態度によって指針が一貫して遵守されていたわけではなかった。本研究から、手袋を脱いだ後の手指衛生を向上させるためには、感染症を減らし、医療従事者の皮膚を保護し、また持続可能性を促進する目的で手袋の使い回しに対する意識を促しながら、標的を絞った介入の必要性が強調される。
