背景

多剤耐性菌、特に基質特異性拡張型β-ラクタマーゼ産生腸内細菌目細菌(ESBL-E)およびカルバペネム耐性腸内細菌目細菌(CRE)による菌血症の影響は大きいにもかかわらず、その疾患負荷については十分に研究されていない。

目的

エアロゾルボックス製品である、吸入システム付きの SplashGuard Caregiver(SGGC)が及ぼす影響を、エアロゾル発生手技後におけるウイルス粒子の存在を直接解析することにより、また数値流体力学(computational fluid dynamics)シミュレーション法を用いることにより検討した。

方法

本前向き観察研究では、集中治療室(ICU)に入室している SARS-CoV-2 感染患者をケアする医療従事者を対象に検討した。治療室は、SGCG ありおよび SGCG なしに分類した。直接解析によりウイルスを検出し、また数値流体力学を用いて、患者の呼吸活動により発生する空中浮遊粒子の動的挙動をシミュレーションした。

結果

実施した解析 67 件中、試験したサンプル 3 個が定量的 PCRにて陽性であり、内訳は SGCG あり群で解析 33 件中サンプル 1 個(3%)、SGCG なし群で解析 34 件中サンプル 2 個(5.9%)が陽性であった。数値流体力学モデルによるシミュレーションから以下が示された:(1)SGCG を構成するプラスチックフィルムとプレキシガラスの間の隙間を小さくすることで、排出された粒子のうち室内に浮遊状態で留まった数が最高70%減少する、(2)医療従事者を主な空気の流れと反対向きに配置すると空気中浮遊粒子への曝露が低減する。

結論

本研究により、SARS-CoV-2 感染患者のいるICU 陰圧室では、SGCG 使用の有無にかかわらず、医療従事者で SARS-CoV-2 の存在が認められた。シミュレーションを行うことは、SGCG のデザインならびに ICU における医療従事者の配置を改善する助けとなるであろう。

監訳者コメント


エアロゾルボックスは患者の頭部に配置し、エアロゾル発生手技時の飛散を抑える装置(実際の製品は論文参照)である。本研究はエアロゾル発生手技後におけるウイルス粒子の存在を直接解析し、流体力学シミュレーション法を用いることにより検討している。この装置の使用にかかわらず、ウイルスの検出を認めたが、医療従事者を空気の流れと反対向き(いわゆる風上)に配置することで感染リスクを低減することはエアロゾルボックスの使用の有無にかかわらず、臨床で意識すべきである。
