Enforcing surveillance of antimicrobial resistance and antibiotic use to drive stewardship: experience in a paediatric setting

背景

小児科における抗菌薬適正使用支援（AS）介入は、方法論、測定基準、転帰についてまだ標準化されていない。我々は、教育と、電子アプリによってガイドラインを提供することに基づく小児科領域の AS 介入の結果を報告する。

材料と方法

AS 介入は、2021 年に、観察、教育、審査とフィードバック、地域の感受性データに基づく抗菌薬の処方を支援するための電子アプリ（Firstline.org）の提供を通して実施した。主要転帰は、介入後 12 か月間の抗菌薬消費量（2022 年）とし、分割時系列分析によって 12 か月間の過去の対照（2019 年）と比較した。副次的転帰は、療法の適切性、入院期間、30 日以内の再入院、小児集中治療室への転室、院内死亡率、抗菌薬耐性（AMR）の出現率とした。

結果

介入後の期間に、467 例の患者を組み入れ、29 回の横断的審査とフィードバックを実施した。処方は、ガイドラインに基づくと 85.7％の症例で適切であり、時が経っても傾向は安定していた。抗菌薬消費量の有意な減少が、介入後の期間に、1,000 患者日当たりの規定 1 日用量（－222.13、P ＜ 0.001）と 1,000 患者日当たりの治療日数（－452.49、P ＜ 0.001）に関して測定され、「Access」/「Watch」比がはっきりと反転した（0.7 から 1.7 まで）。入院期間、院内死亡率、集中治療室への転室、AMR であった感染の発生率は、安定したままであったが、30 日以内の再入院は、入院 100 件当たり 4.9 から 2.8 に減少した（P ＜ 0.001）。

結論

介入は、抗微生物薬消費の有意な減少、および処方の適切性の増加と関連していた。電子ツールは、ガイドラインへのアドヒアランスを促進し、結果を確実に維持するのに有用である可能性がある。
