Game-based training to improve the compliance of hospital pharmacy operators with handwashing guidelines

緒言

世界保健機関（WHO）が示すような適切な手指衛生の方法により、無菌製剤の微生物汚染が防止される。本研究の目的は、病院薬剤師の手指衛生ガイドライン遵守を改善するために、ゲームに基づくトレーニングツール（Handtastic Box）の有効性を評価することである。

方法

トレーニング開始前 1 か月間、トレーニング開始後 1 か月間（M1）、その後 1 か月から 3 か月まで（M2＆3）、病院薬剤師の手洗いをカメラで記録した。それらの動作について、完全に実施、部分的に実施、非実施としてスコア化した。病院薬剤師それぞれの手指衛生技術の遵守率を観察期間で比較した。1 時間のトレーニングセッションにおいて、トレーニングを受ける病院薬剤師が二人一組で手洗いのビデオを視聴し、ガイドラインの 5 つのどのステップが抜けているかに注目した。フルオレセインで染色された複数の箇所がある木製手指を紫外線下に調べ、一致する汚染を示す手指を探し出した。

結果

病院薬剤師 9 例の平均遵守率は、44.6％（トレーニング前、ビデオ数 32）から86.7％（M1、ビデオ数 40）、82.5％（M2＆3、ビデオ数 45）に増加した。ステップごとの遵守率は、トレーニング前から M1 まで改善し、M2＆3 では、この期間に著しく低下した指先端の洗浄ステップを除き全体的に安定していた。

考察

著者の知る限り、本研究は病院薬剤師を対象に、WHO 手指衛生方法の遵守の改善を図るゲームに基づくトレーニングの有効性を評価した最初の研究である。このツールは手洗いスコアを著しく改善し、トレーニング後 3 か月間、スコアは同程度に維持された。各ステップの個々の結果は、動作ごとにトレーニングする必要性を強調するものである。

結論

ゲームに基づくトレーニングツールにより、3 か月にわたり WHO 手指衛生方法の遵守を改善することに成功した。このトレーニングは、他の医療従事者にも使用できるであろう。
