集中治療室におけるメチシリン耐性黄色ブドウ球菌(Staphylococcus aureus)の伝播の予防に対する接触予防策の効果:現地試験のレビューおよびモデリング研究★

背景
接触予防策(CP)は、病院、特に集中治療室(ICU)において、メチシリン耐性黄色ブドウ球菌(MRSA)に対処する標準的な手法として、広く実施されてきた。しかし、MRSA の伝播の予防に関する CP の有効性は、依然として議論の対象である。
目的
ICU 内で MRSA の伝播を減少させることに対する CP の有効性を評価すること。
方法
ICU における CP の有効性を調べた 13 件の現地試験から収集した 16 セットのパラメーターからのデータを使い、包括的な解析を実施し、病院における MRSA の発生の動態をシミュレーションする疫学的モデルを開発した。
結果
解析では、MRSA 伝播率の平均 20.52%(95%信頼区間 -30.52 ~ 71.60%)の減少が CP の実施と関連していたことが明らかになった。さらに、スクリーニング検査の時間の遅れを減少させたことと、入院時のスクリーニングで MRSA の存在が検出される患者の割合を増加させたことが、CP の有効性に貢献していたことが分かった。
結論
適切な CP の実施、特に、MRSA が定着/感染している患者を早期に特定することに重点を置いた実施が、MRSA の病院内での伝播の減少と強い関連を示した。しかし、定着している患者における感染のリスクを軽減するために CP と併用される除菌やその他の介入の有効性と安全性を調べるには、更に研究が必要である。
監訳者コメント:
MRSA 感染対策における接触予防策(CP)の効果をみたレビューであるが CP の実施により一定の効果はありそうであるが、 CP や手指衛生の遵守率、さらに CHG 入浴による除菌などは大きく MRSA 感染拡大に影響を与える因子であり、これらを無視することはできない。すでに MRSA や VRE に対して CP を中止している施設では手指衛生の高い遵守率や CHG 入浴等の介入がある。入院時のスクリーニングでは結果報告が感染拡大に関与するため培養よりも核酸増幅法が効果的、入院時の陽性者が少なく入院期間が短ければ CP の効果は認めにくい、また CP を中止しても保菌率は上昇するが MRSA 感染症発生率に変化ないなどが指摘されているが、 MRSA と CP の効果についてはまだまだ研究の余地が残されている。
