ヒトの抗菌薬の処方に影響を与える因子を明らかにする：Theoretical Domains Framework から情報の提供を受けたメタ統合研究

この研究は、ヒトの医学における、臨床での抗菌薬の処方の選択に関係する変更可能な決定因子（促進因子と障壁因子）を特定することを目的とした。抗菌薬の管理を強化することは、耐性菌の広がりを遅らせるのに役立つ可能性がある。Sandelowski と Barroso の方法に従い、定性的メタ統合の手法を使った。組み入れた研究は、曖昧さ回避プログラム（Critical Appraisal Skills Programme）を使い評価した。研究結果を抽出し、まとめて、定性的なメタサマリーを作成した。理論的ドメインのフレームワーク（Theoretical Domains Framework）の能力－機会－モチベーション（Capabilitiese–Opportunitiese–Motivation, COM-B）モデルと Behaviour Change Wheel を、データの解釈のための coding matrix として使った。組み入れた 63 件の研究の解析では、理論的ドメインのフレームワークで挙げられている 14 個のドメインのうち 12 個で、障壁因子と促進因子が明らかになった。処方者の能力、意欲、機会が、抗菌薬の処方行動の主要な推進因子であることが分かった。知識、技能、信条、期待、患者や同僚の影響、組織文化、下部構造の特徴が、処方行動に有意な影響を与える。抗菌薬の処方に関係する因子の網羅的な目録を編集した。抗菌薬の適切な処方を促進するための介入では、個人のレベルの変数にのみ焦点を当てるよりも、系統的な手法をとるべきである。さらに、そのような介入では、組織の実際の状況における適用可能性と持続可能性を確実に高めるために、コデザインの手法を採用することが望ましい。
監訳者コメント：
抗菌薬の処方の処方は、数学で割り切れるほどきっちりした処方プロトコールに依存できれば良いが臨床的には困難な状況もある。こうしたときにどういった手法を用いるのかが大きな問題となる。
