背景
長期介護施設（LTCF）では、COVID-19 パンデミックの早期に感染者が多くでたが、新型コロナウイルスの影響は、ワクチン接種キャンペーンや過去の感染による免疫の蓄積により、時間の経過とともに小さくなった。
目的
2020 年 10 月から 2023 年 3 月にわたる VIVALDI 研究の実施期間中に、イングランドの LTCF における SARS-CoV-2 に関連する死亡および入院の発生率について評価すること。
方法
本研究に参加した LTCF から、追跡調査期間が研究解析期間内に含まれていた 65 歳以上の入居者を組み入れた。COVID-19 に関連付けられた死亡および入院の発生率比を暦年四半期ごとに算出し、あわせて SARS-CoV-2 検査陽性後の感染致死率比（28 日以内）および感染入院率比（14 日以内）を算出した。
結果
入居者計 26,286 例を組み入れ、8,513 例（32.4％）で少なくとも 1 回の SARS-CoV-2 検査陽性が認められた。COVID-19 関連死亡の発生率比は、2021 年の第 1 四半期に 1,000人日あたり 0.47 （全死亡数の約 3 分の 1）でピークが認められたのに対し、2023 年の第 1 四半期には1,000人日あたり 0.10 であり、これは SARS-CoV-2 感染症の発生率比と同程度であった。SARS-CoV-2 感染症による感染致死率比には、上記期間内において 24.9％ から 6.7％ への低下が、感染入院率比には 12.1％から 8.8％への低下が観察された。この対象集団では、評価した各四半期における全死亡率比が高く、28.8 ～ 41.3％という年間死亡確率に対応していた。
結論
LTCF における感染獲得後の入院および死亡の、標準化を行うリアルタイムモニタリングから、感染伝播を予防するための非薬理学的介入の必要性に関する方針のための情報が得られるであろう。

イングランドでの長期介護施設における COVID-19 に関連する死亡および入院に関して、2020 年 10 月から2023 年 3 月まで 3 か月毎に影響をモニタリングした研究。ワクチン接種や過去の感染の蓄積に伴って COVID-19 の影響は経時的に少なくなっている。このようなモニタリングをリアルタイムに行うことで、施設への訪問制限、無症状の入所者や職員への検査、マスク着用の義務化などの感染対策の強化や緩和の方針決定に役立ちそうである。
