結果が「おそらく」の場合：SARS-CoV-2 の不確定な結果とその関連性に関する後向きレビュー

RT-PCR法を用いたSARS-CoV-2の検査では、不確定な結果が発生する可能性があり（サイクル閾値 30 以上）、臨床的重要性を決定するためにさらなる検査を必要とする。不確定な結果の後に決定的な結果（「検出」／「未検出」）を予測できる患者変数は、病床管理の最適化および限られた資源の利用に有用であろう。2020 年 3 月から 2022 年 3 月にアイルランドの 3 次病院で、SARS-CoV-2 の不確定な結果に関する後向き観察研究を実施し、人口統計学的特性、併存疾患、免疫抑制がその後の検査での決定的な結果と関連するか否かを明らかにした。データは患者および検査記録から入手した。関連性の分析を Fisher 正確検定により実施し、予測可能性をロジスティック回帰により検証した。初回検査で不確定な結果であった患者 411 例のうち 299 例（72.2％）が、その後の検査で決定的な結果であることが示され、29 例が「検出」、270 例が「未検出」とされた。「検出」群では、COVID-19 の診断歴が、「検出」となるリスクの低下と関連した（粗オッズ比 ＝ 0.10、95％信頼区間［CI］0.03 ～ 0.35）。「未検出」群では、ワクチン接種を受けた患者が、その後の検査で「未検出」となる可能性が低かった（補正オッズ比 ＝ 0.57、95％CI 0.34 ～ 0.94）。不確定な結果の調査では、COVID-19 感染歴のある患者は、「検出」という結果になる可能性が低く、ワクチン接種患者は、「未検出」となる可能性が低かった。本研究は、SARS-CoV-2 の管理における十分な臨床病歴の必要性を強調するものである。
監訳者コメント：
RT-PCR 法でのSARS-CoV-2の検査は、COVID-19 の検査として最も早期より検出される。一方で高感度故の陽性期間のついても慎重に見極める必要がある。
