背景

新生児集中治療室(NICU)においてメチシリン感性黄色ブドウ球菌(meticillin-sensitive Staphylococcus aureus ; MSSA)感染症は、健康上の重大な懸念となっている。保菌状態は、その後の感染症のリスクを高め、罹病および死亡につながる。

目的

NICU 患者における MSSA の保菌および感染のサーベイランスに関する後向きコホート研究の結果を報告すること。

方法

咽頭、鼻、肛門、鼠径部の MSSA の保菌、ならびに侵襲性および非侵襲性の MSSA 感染症に関して、2020 年 11 月から 2022 年 6 月までの週 1 回の微生物学的サーベイランスの結果を分析した。ステップワイズロジスティック回帰分析により交絡因子を補正し、MSSA 感染症と保菌リスクを比較した。

結果

スクリーニングされた新生児 383 例のうち 42.8%(164 例)が MSSA の保菌状態にあった。MSSA の保菌の有意なリスク因子は、入院期間、経腟分娩、在胎 28 週未満で出生した超早産であった(すべて P < 0.05)。サーベイランスにより、軽症 MSSA 感染症 38 例(9.9%)、侵襲性 MSSA 感染症 11 例(2.9%)が認められた。全体的に見て、新生児の MSSA の保菌は、MSSA 感染症の主要なリスク因子であり(保菌/感染 29.3% 対 保菌/非感染 70.7%、非保菌/感染 0.5% 対 非保菌/非感染 99.5%)、また侵襲性 MSSA 感染症も同様であった(保菌/感染 6.1% 対 保菌/非感染 93.9%、非保菌/感染 0.5% 対 非保菌/非感染 99.5%)。さらに、超低出生体重(1,000 g 未満)、超早産、侵襲的人工換気は MSSA 感染症の有意なリスク因子であった(すべて P < 0.05)。

結論

出生後の MSSA 保菌とその後の MSSA 感染症との関連は、予防に向けた可能性を提示するものである。経腟分娩と新生児 MSSA 保菌の原因との関連を検討するために、さらなる研究が必要である。

黄色ブドウ球菌は、表皮ブドウ球菌のような健常児での保菌はない。それがメチシリン感性黄色ブドウ球菌(MSSA)であろうと、メチシリン耐性黄色ブドウ球菌(MRSA)であろうと病原性菌であることに注意が必要である。
