背景

この研究の目的は、ヨーロッパの施設における股関節や膝関節の最初の関節形成術後の黄色ブドウ球菌(Staphylococcus aureus)による人工関節感染(SA-PJI)の発生率、関連する疾患負荷、医療の利用を推定することであった。

方法

この研究は、2014 から 2016 年にヨーロッパの 19 の病院で、股関節や膝関節の最初の関節形成術を受けた患者を対象に実施した。人工関節感染(PJI)および SA-PJI の全体での発生率を計算した。関連する疾患負荷は、感染に関係した死亡 + 機能の喪失として、間接的に測定した。医療の利用については、入院の回数と期間、外科処置の回数と種類、抗菌薬による治療の期間、外来の受診回数を収集した。サブグループ解析と回帰分析を使って、SA-PJI の医療の利用に対する影響を評価した。その際、交絡変数について調整した。

結果

微生物による PJI の発生率は、1.41%で、SA-PJI は 0.40%であった。SA-PJI では、20.7%が MRSA によるもので、かなりの地域差があり、股関節の部分形成術で頻度が高くなっていた。関係した死亡と機能の喪失は、SA-PJI の症例のそれぞれ 7.0%と 10.2%に発生し、股関節の部分形成術の患者の方が発生率が高かった。PJI のなかった患者と比べて、SA-PJI の患者は、交絡変数について調整後、再入院が平均で 1.4 回多く、入院が 25.1 日多く、受けた外科処置が 1.8 回多く、外来受診が 5.4 回多かった。医療の利用は、SA-PJI の外科的治療が失敗した患者の方が多かった。

結論

この研究は、SA-PJI の負荷が高いこと、特に股関節の部分形成術で高いことを確認した。そして、SA-PJI を予防するための介入を計画するための確固たる基礎をもたらした。
