背景

病院感染(HAI)、ならびに薬剤耐性(AMR)を示す感染性微生物は、世界的な課題である。環境内で患者が接する排水設備、例えば手洗いのシンク、シャワーおよびトイレなどは、感染性微生物および AMR 遺伝子の感染源として同定されることがますます多くなっている。

目的

多剤耐性微生物による HAI アウトブレイクが発生したアイルランド共和国の大規模教育病院 1 施設の排水システムについて、大規模メタゲノム解析の結果を提示すること。

方法

HAI が流行していた内科病棟の改修直前に、排水パイプ部分(N = 20)を取り外した。これらは、トイレの防臭弁、ならびにシンクとシャワーの排水管であった。DNAを抽出した後、各パイプ部分についてメタゲノム解析を実施した。

結果

多様な分類学的およびレジストームのプロファイルが観察され、プロテオバクテリア門およびアクチノバクテリア門が優勢であった(それぞれ 38.23 ± 5.68%および 15.78 ± 3.53%)。臨床分離株 5 株についてゲノム解析を行った。これらの AMR 細菌分離株は、病棟に入院後 48 時間を超えた患者から分離された。ゲノム解析から、これらの分離株は多数の AMR 遺伝子を保有していることが分かった。

結論

分離株のレジストームプロファイルと排水システムのメタゲノムを比較したところ、高度の類似性が示され、多くの同一の AMR 遺伝子が共通に認められ、これらにより入院後に獲得された可能性が高いことが示唆された。最も多く認められた AMR 遺伝子は、臨床的に重要かつ高頻度に用いられている抗菌薬クラスに対する耐性をコードする遺伝子であった。平均ヌクレオチド同一性分析から、臨床分離株と排水パイプにおいて高い類似性または同一性のゲノムが存在することが確認された。これら独自の大規模解析から、院内感染症の伝播を予防するため、また可能性のある排水システム内のリザーバにおける AMR の発現を予防するために、患者が接する病院内の排水パイプに対する定期的な清掃および汚染除去ならびに効果的な感染制御方針が必要であることが強調される。
