液体加温装置の熱伝導流体として使用される水の細菌汚染★★

緒言
最近の報告では、輸液や血液の加温、または体外式膜型人工肺装置における加温のために使用されるヒータークーラーユニットが、非結核性抗酸菌など潜在的な病原菌による医療関連感染症(HAI)の原因になりうるとされている。これは、通常は無菌状態の設備にある汚染源を意味している。
目的
輸液加温装置の水の細菌汚染について分析することと、輸液加温装置が HAI 伝播の潜在的な発生源であるかどうかを明らかにすること。
方法
別個の 22 の輸液加温装置のリザーバから熱伝導流体(300 ~ 500 mL)を回収し、コロニー数計測と細菌同定のために、各種の選択培地および非選択培地上で処理した。マイコバクテリウム(Mycobacterium)属菌株を全ゲノムシークエンシングにより解析した。
結果
22°C、36°Cで培養後、熱伝導流体の 22 サンプルすべてに細菌増殖が観察された。同定された病原体で最も多かったのは緑膿菌で、サンプルの 13.64%(22 サンプル中 3 サンプル)を占め、100 CFU(コロニー形成単位)/ mL 超が検出された。マイコバクテリウム・キマイラ(Mycobacterium chimaera)、ラルストニア・ピッケティ(Ralstonia pickettii)、ラルストニア・マンニトリリティカ(Ralstonia mannitolilytica)が、サンプルの 9.09%(22 サンプル中 2 サンプル)で検出された。検出された M. chimaera の一次シークエンシングでは、患者 2 例が死亡したスイスでのアウトブレイク時に検出された M. chimaera 株と近縁であることが示唆された。
考察
感受性の高い患者のいる部署では熱伝導流体の汚染は病原体の供給源となりうる。輸液加温装置の誤操作は、日和見性病原体または通性嫌気性細菌などの病原体の分布につながり、院内感染症伝播のリスクを高める可能性がある。
監訳者コメント:
周術期に体温維持のために体温加温冷却装置が使用されるが、この装置内の液体汚染により術後心内膜炎の死亡事例が 2 例報告され、起炎菌として非結核性抗酸菌 M.chimaera が同定されている。装置の液体と手術室の空気中から M.chimaera が検出され、空気感染による術後創部感染と判断された。加温装置は手術時の体温調節のみならず、輸血製剤の加温などでも幅広く使用されているが、患者に直接的な接触がない形で熱伝導がおこなわれており、感染のリスクは低いと考えられていた。これらに使用される水は無菌であることが要求されているが、時に使用中に汚染されあるいは水道水を使用することで汚染が発生している。これらの装置の取扱いについて再度見直しが必要である。
