背景

臨床の現場では、表面の消毒が、微生物の交差感染のリスクや、院内感染のリスクを減少させる対策として重要である。標準化された方法を使って、消毒法を評価し、消毒に使われる活性成分の有効性を評価することができる。しかし、標準化にもかかわらず、そのような方法による結果は、依然として、いくつかの要因によって決定される。そして、結果が誤っていると、消毒剤の有効性についての推測が妥当なものではなくなる可能性があり、それは、患者や医療従事者の健康に対する重大なリスクとなる。

目的

この研究の目的は、緑膿菌(Pseudomonas aeruginosa)やその他の試験用微生物の回収におけるいくつかの決定要因を評価し、標準化された消毒法による結果に対するそれらの影響を明らかにすること、および緑膿菌に替わって使用できる適切なグラム陰性菌株を見つけることであった。

方法

試験用微生物の生存と回収に対する接種方法、乾燥時間、温度、キャリア材料の影響を、スチューデントの t 検定、一元配置分散分析、テューキーの多重比較検定を使い、評価した。

結果と結論

温度、乾燥時間、接種方法、キャリア材料が緑膿菌細胞の回収に有意に影響を与えていたために、使用した方法による結果に影響を与えたことが分かった。この研究は、緑膿菌をグラム陰性の菌種であるアシネトバクター・バウマニー(Acinetobacter baumannii)に替えることができることも示した。A. baumannii は、標準化された多くの方法で使われている試験用微生物の 1 つで、同じ状況での反応が緑膿菌より良く、消毒剤有効性検査における振る舞いが緑膿菌に似ている。

監訳者コメント


消毒剤の効果は、温度、濃度、接触時間により決定されることは周知の事実であるが、実際の現場での使用状況は実験条件とは異なるため、その差を知っておく必要がある。乾燥により菌が死滅する場合には、正確な消毒剤の効果が判断できないこととなるし、菌が付着する材質やその環境温度も大きく影響する。回収率は大きく菌の乾燥抵抗性に依存する。特に緑膿菌は乾燥に影響を受けるにもかかわらず、EN 16615:2015での試験のための標準菌として使用されているのは、病院施設の湿潤環境に広く生息し院内感染の代表菌であることが大きな理由であろう。一方、アシネトバクターは黄色ブドウ球菌と同程度の乾燥抵抗性があり、病院内で拡大しやすく、多剤耐性アシネトバクターの感染拡大が世界的にも懸念されており、グラム陰性菌の試験用の標準株として消毒剤の効果判定に使用することは有用であろう。
