東アフリカ・タンザニアの 3 次紹介病院の整形外科患者および環境から得たβ-ラクタマーゼ産生アシネトバクター・バウマニー（Acinetobacter baumannii）の一部における病原性遺伝子およびプラスミドレプリコンのプロファイル

アシネトバクター・バウマニー（Acinetobacter baumannii）は、多剤および消毒薬に高い耐性を示すことに加え、病院環境で存続する能力をもつことから、重要な院内病原菌となっている。タンザニアの 3 次紹介病院で入院中の整形外科患者から直腸スワブを採取し、β-ラクタマーゼ産生 A. Baumannii のスクリーニングを行った。患者の介護者、医療従事者、隣接する無生物環境からもスワブを採取した。確定したβ-ラクタマーゼ産生 A. Baumannii 計 26 株が分離され、このうち代表的な分離株 4 株（2 株は患者から、2 株は病院環境から）の全ゲノムシークエンシング（WGS）を行い、配列型（ST）、β-ラクタマーゼ遺伝子、プラスミドレプリコン型、病原性遺伝子を検出した。分離株 4 株すべてが、blaADC-25(3)、blaOXA(4)、blaCTX-M-15(2)、blaNDM-1(2) などのβ-ラクタマーゼ遺伝子を複数有した。分離株はさらに、以下をコードする病原性遺伝子を有した。外膜タンパク質（ompA）、curli タンパク質（csg）、シデロフォア生合成系（エンテロバクチン［entABCDEFS、fepABCDG、fes］、エルシニアバクチン［ybtAEPQSTUX、irp1、irp2、fyuA］、エアロバクチン［iucABCD、iutA］）、輸送分泌系 II 型（T2SS）および III 型（T3SS）、E. coli common pilus（ecpRABCDEオペロン）、1 型線毛（fim）、アリールスルファターゼ（aslA）および接着（fedC）。患者に由来する分離株のみが、それぞれ 4 つのプラスミドレプリコンを有し、最も多く認められたプラスミドレプリコンは IncFIA_1、IncY_1 および IncFIB(AP001918)_1 であった。入院中の整形外科患者および病院環境は、病原性遺伝子が備わった、複数のβ-ラクタマーゼ産生 A. baumannii（blaOXA や blaNDM-1 のような、カルバペネム系薬に対して作用する細菌を含む）のリザーバとして働くことから、病院環境内での院内感染の伝播を抑制するためには、入院時に開放骨折があった整形外科患者の定期的スクリーニングと、感染予防・制御策の強化が必要であることが強調される。
監訳者コメント：
地域で流行しているβ-ラクタマーゼ産生 Acinetobacter baumannii を把握し、疫学情報としてもち合わせるとともに、施設内における院内感染を予防するのは大変重要である。
