Length of intensive care unit stay and the apparent efficacy of antimicrobial-based versus non-antimicrobial-based ventilator pneumonia prevention interventions within the Cochrane review database

背景

人工呼吸器関連肺炎（VAP）感染のリスクは、集中治療室（ICU）への入室期間（LOS）に伴い増加する。本研究の目的は、非抗菌薬介入と抗菌薬介入を比較した無作為化同時対照試験から得られたデータを用いて、第一に明らかな VAP 予防効果、第二に対照群と介入群の VAP 発生率と、LOS との関連を推定することである。

方法

VAP 予防介入の無作為化同時対照試験において、抗菌薬ベースの試験 78 件と非抗菌薬ベースのさまざまな試験 111 件に関する 13 の Cochrane レビューから、対照群と介入群のデータを入手した。

結果

VAP 予防効果 対 各群の平均 LOS のメタ回帰モデルにおいて、非抗菌薬ベースの介入群の効果量はゼロに近づき（＋0.028、95％信頼区間［CI］＋0.002 ～ 0.054）、一方、抗菌薬ベースの介入群ではゼロから離れた（－0.043、95％CI －0.08 ～ －0.004）。9 日目 ～ 10 日目の VAP 発生率が、抗菌薬ベースの対照群では 1 日あたり 1.28（95％CI 0.97 ～ 1.6）パーセンテージポイント増加したのに対して、抗菌薬ベースの介入群（0.45、95％CI 0.19 ～ 0.71）、非抗菌薬ベースの介入群（0.58、95％CI 0.29 ～ 0.87）、非抗菌薬ベースの対照群（0.76、95％CI 0.46 ～ 1.05）では、同期間の VAP 発生率の増加はすべて同程度であった。

結論

抗菌薬ベースの介入と非抗菌薬ベースの介入の比較では、全体的に明らかな VAP 予防効果が大きく、これは平均 LOS が長い群ほど顕著であった。この驚くべき LOS との関連の理由は、逆説的ではあるが、介入群よりむしろ対照群内にある。この矛盾、間接的（波及）効果、すなわち個々の抗菌薬ベースの無作為化同時対照試験内における不顕性を示唆しており、VAP 予防効果の顕れが混同されている可能性がある。

監訳者コメント：

ICU での在院日数が VAP のリスクとなることは周知の事実であり、 5 日目が発生のピークとなる。コクランレビューでは、抗菌薬介入（局所投与や全身投与に関係なく）により VAP の発生率が最大 50％まで低下し、一方非抗菌薬介入では VAP 発生率は下げないかまたは軽度の低下に留るとの結果である。VAP の予防効果の検討においては介入内容、対象患者、VAP 診断基準、研究の質や研究デザインなど、夫々の条件が異なるためメタ解析を難しくしている。
