脊椎インストゥルメンテーション手術後の化膿性脊椎炎:リスク因子と管理★

背景
脊椎インストゥルメンテーション手術後の化膿性脊椎炎(pVOM)は、まれな合併症である。ほとんどの感染症例は皮膚と軟部組織が関与する手術の後、早期に発生し、デブリードマンと抗菌化学療法によるインプラント温存(DAIR)で管理できる。
目的
pVOM のリスク因子を特定し、管理戦略を評価すること。
方法
2010 年から 2016 年の脊椎インストゥルメンテーション後の深部感染症(IASI)症例の多施設共同コホートから、pVOM 症例を脊椎が関与していない症例と比較した。早期感染を術後 60 日未満の場合、晩期感染を術後 60 日を超える場合と定義した。多変量解析を用いてリスク因子を調査した。
結果
IASI 症例 410 例のうち、19 例(4.6%)が pVOM を発現し、早期 IASI の 2%(7/347)から晩期 IASI の 19.1%(12/63)に及んだ。多変量解析後も、年齢(補正オッズ比[aOR]1.10、95%信頼区間[CI]1.03 ~ 1.18)、椎体間固定術(aOR 6.96、95%CI 2 ~ 24.18)、コアグラーゼ陰性ブドウ球菌(CoNS)感染症(aOR 3.83、95%CI 1.01~ 14.53)は、pVOM の独立したリスク因子のままであった。pVOM を有する症例は、有さない症例よりも予後が悪かった(失敗率 26.3%対 10.8%、P = 0.038)。材料の抜去は、主に早期症例において、転帰が良好でない場合に推奨される戦略(57.9%)であった(失敗率 33.3%対 50%、DAIR と比較)。抜去によって管理された晩期症例は、DAIR と比較してはるかに成功した(失敗率 0%対 40%、P = 0.067)。
結論
pVOM のリスク因子は、高齢、椎体間固定術のデバイスの使用、CoNS の病因である。診断は予後不良につながるが、晩期症例に対してまたは脊椎固定術を達成した場合には、材料の抜去を予定しておくべきである。

脊椎インストゥルメンテーション手術後の化膿性脊椎炎のリスク因子と管理についてのスペインでの多施設共同後向き観察研究。これまで報告されていなかった pVOM のリスク因子が特定されている。また、pVOM に対する抗菌薬投与期間についても結論は出せないが、考察されている。
