Pulling the plug on a pseudomonas outbreak: ancillary equipment as vectors of infection

目的

軟性内視鏡に関連する感染症のアウトブレイクについては、十分に記述されている。しかし、軟性内視鏡は灌流プラグなどの補助的器具の使用も必要とする。これらは感染症の潜在的なベクターであるが、文献で強調されることはほとんどない。本稿では、英国の 3 次医療センター 1 施設における汚染された灌流プラグ由来の緑膿菌（Pseudomonas aeruginosa）による膀胱鏡関連のアウトブレイク 1 件について報告する。

方法

検査室、臨床および汚染除去室の記録を再調査し、汚染除去室の監査を行った。軟性膀胱鏡および灌流プラグの汚染について評価した。微生物学的検査情報管理システムを用いて、後向きおよび前向き症例発見を行った。緑膿菌の入手可能な分離株に対して、variable number tandem repeat（VNTR）タイピングを行った。確定症例は、アウトブレイク株と同一の VNTR プロファイルを有する緑膿菌感染症と定義した。

結果

5 つの灌流プラグから緑膿菌 3 株が分離されたが、軟性膀胱鏡からは分離されなかった。獲得された耐性メカニズムは検出されなかった。菌血症、化膿性関節炎、尿路感染症を含む確定した感染症 15 件が発生した。汚染除去の失敗は再処理前に灌流プラグを分解しなかったために発生した可能性が高い一方で、製造者による再処理の説明書も英国の標準的な手順に適合していなかった。英国医薬品・医療製品規制庁に情報提供した。市場安全性通知が発行され、製造者は再処理説明書の改訂版を発行した。

結論

補助的器具は感染症の重要なベクターとなる可能性があり、アウトブレイク調査時には考慮すべきである。使用者は、利用可能な手順と適合していることを確認するため、補助的器具の再処理に関する製造者の説明書を見直すべきである。
