エムポックス汚染からの医療従事者の防護：French Society for Hospital Hygiene のポジションペーパー

近年の世界的なエムポックスの再興を受け、French Society for Hospital Hygiene（SF2H）は伝播経路に関する文献レビューを実施し、エムポックスウイルス感染が疑われるまたは確定した患者のケアを行う医療従事者向けの具体的な推奨事項を提案した。
先進国では、医療施設内での医療従事者の汚染リスクは極めて低いと考えられ、針刺し切創による汚染に限定された。その他に 2 例の汚染例が報告されたが、十分に説明されなかった。医療環境以外では、文献の解析から、（i）主に男性間性交渉者における性交時の主要汚染経路、（ii）学校や刑務所などのその他の環境における極めて低い二次発症率が明らかになった。多数の研究において、患者周囲の表面上または空気中での分子またはウイルスの同定が報告されているが、低い二次症例発生率との関連は認められない。 さらに、空気または粘膜曝露を介する最小感染量は依然として不明である。医療環境におけるエムポックスウイルスの呼吸器伝播のエビデンスはないため、SF2H は、医療用／サージカルマスクの使用と組み合わせて標準的な接触予防策を実施することを推奨している。経皮汚染のエビデンスはないため、SF2H は、皮膚病変または粘膜との接触が起こる場合のみ手袋を着用することを推奨している。医療施設内の環境からの汚染リスクについては、調査のためにさらなる研究を実施する必要がある。
監訳者コメント：
エムポックスについては、かたまった集団のなかでどのような伝播がヒト・ヒト間で起こるのか、「空気または粘膜曝露を介する最小感染量は依然として不明」と用心深く対応をするように著者は求めている。疫学解析から、もうしばらくするとその頻度がわかってくるだろう。
