The intertwining of healthcare-associated infections and COVID-19 in Italian intensive care units: an analysis of the SPIN-UTI project from 2006 to 2021

背景

医療関連感染（HAI）は、公衆衛生に対する並はずれた負荷となるが、新型コロナウイルス感染症（COVID-19）の影響についてはまだ議論されている。

目的

2006 年から 2021 年のイタリアの集中治療室（ICU）における HAI の傾向を記述することと、COVID-19 のあった患者となかった患者の特徴および転帰を比較すること。

方法

「Italian Nosocomial Infections Surveillance in Intensive Care Units」（SPIN-UTI）プロジェクトに参加した患者を評価した。これらの患者は ICU 入室時間が48 時間を超えていた。COVID-19 患者の診断、臨床状態、治療法、転帰に関するデータも収集した。

結果

2006 年から 2021 年の合計 21,523 例の患者のうち、3,485 例（16.2％）に、少なくとも 1 件の HAI があった。HAI 患者の発生率と HAI の発生密度の両方の増加傾向が観察された（P_傾向 ＜ 0.001）。パンデミック前の時期と比べて、HAI の発生密度は 2020 年から 2021 年に約 15％増加し、この増加に最も貢献したのは肺炎であった（P_傾向 ＜ 0.001）。さらに、HAI の発生率は、COVID-19 患者専用の ICU の方が高く（P ＜ 0.001）、COVID-19 患者は、COVID-19 のなかった患者より HAI および死亡のリスクが高かった（P値 ＜ 0.001）。したがって、ICU での死亡率は、年々増加し、パンデミック中に 2 倍となった（P_傾向 ＜ 0.001）。特に、同時感染の患者は、死亡率（75.2％）が、COVID-19 のみ（66.2％）または HAI のみ（39.9％）の患者や、感染のなかった患者（23.2％）より高かった。

結論

我々の解析結果は、COVID-19 パンデミックがイタリアの ICU におけるHAI の発生と死亡に影響を与えたかどうか、またどのように与えたかについて有用な理解をもたらし、パンデミックの長期的な影響を評価することが必要であることをはっきりと示している。
