新生児集中治療室におけるカテーテルの種類別のカテーテル関連血流感染症のリスク：1,100 本超の血管内留置カテーテルに関する大規模コホート研究

背景
本研究の目的は、スイスの 3 次医療センターの新生児集中治療室（NICU）における大規模な前向きデータベースを用いて、各種カテーテル間でカテーテル関連血流感染症（CABSI）のリスクを評価することである。
方法
2017 年 1 月から 2020 年 12 月までに NICU に入室し、中心血管内留置カテーテルを 1 回以上挿入されたすべての新生児を対象とした。各種カテーテルにおける CABSI リスクを明らかにするために周辺構造 Cox モデルを用いた。
結果
計 574 例の新生児および血管内カテーテル 1,103 本（臍静脈カテーテル 581 本、臍動脈カテーテル 198 本、末梢挿入式中心静脈カテーテル［PICC］324 本）を本研究の対象とした。臍静脈カテーテル、臍動脈カテーテル、PICC を挿入された新生児において、それぞれ 17 件、4 件、4 件の CABSI が特定された。臍帯カテーテル留置 2 日後に CABSI リスクが増大した。性別と在胎期間で補正した単変量 Cox モデルにおいて、臍静脈カテーテルと臍動脈カテーテル間での CABSI リスクは同程度であった（ハザード比［HR］0.57、95％信頼区間［CI］0.16 ～ 2.08）。出生時体重は CABSI と関連し、高体重は防御的因子であった（HR 0.37、95％CI 0.16 ～ 0.81）。
結論
特に低出生体重および超低出生体重の新生児において、臍帯カテーテルの留置期間の短縮を目的とした対策は、この集団の CABSI 発生率の低減のために有効であろう。
