Accuracy of T2 magnetic resonance assays as point-of-care methods in the intensive care unit

背景

菌血症患者における病原体の同定を促進するため、新しい分子診断法が検討されている。

目的

集中治療室におけるポイント・オブ・ケア検査としての T2 核磁気共鳴（T2MR）アッセイ（T2 細菌［T2B］および T2 耐性［T2R］）の実現可能性および診断精度を、血液培養ベースの検査と比較して評価すること。

方法

菌血症の疑いのある連続症例に関する前向き横断研究。参照方法として血液培養を用いて診断精度を評価した。

結果

計 208 例を本研究の対象とした。T2MR アッセイは血液培養ベースの方法と比較して、サンプル採取から報告までの平均時間が短かった（P ＜ 0.001）。無効な報告の割合は、T2B アッセイでは 6.73％、T2R アッセイでは 9.9％であった。T2B アッセイでは、全体の陽性一致率（PPA）は 84.6％（95％信頼区間［CI］71.9 ～ 93.1％）、陰性一致率（NPA）は 64.3％（95％CI 55.4 ～ 72.6％）、陽性予測値（PPV）は 48.9％（95％CI 42.5 ～ 55.3％）、陰性予測値（NPV）は 91.2％（95％CI 84.4 ～ 95.2％）であった。Cohen のκ係数は 0.402 であった。T2R アッセイでは、全体の PPA は 80％（95％CI 51.9 ～ 95.7％）、NPA は 69.2％（95％CI 54.9 ～ 81.3％）、PPV は 42.9％（95％CI 31.7 ～ 54.8％）、NPV は 92.3％（95％CI 81.1 ～ 97.1％）であった。Cohen のκ係数は 0.376 であった。

結論

T2MR アッセイは菌血症の迅速な除外に関して NPV が高く、集中治療室におけるポイント・オブ・ケア診断検査として適用した場合、潜在的に抗菌薬適正使用支援に役立つ可能性がある。
